Making a Gift of Stock to PAWS

Thank you for considering a gift to PAWS! Your donation will make a significant difference for the animals in care.

The following is information you will need to relay to your broker for your stock or other securities to be transferred to PAWS:

• Our broker is Smith Barney
• Our account number is #135-895562-182
• Our DTC number is 0015
• Our agent at Smith Barney is Brian Chase, and he can be reached at 206.343.2766

Once you have requested the transfer, please have your broker carbon copy PAWS on the specifics so that we can properly acknowledge you and provide you with a tax receipt.

We ask that you provide us with the following information, so that we issue a tax receipt and acknowledgment:

• Your name(s)
• Your address
• Your phone number and e-mail
• The number of shares and the name of the securities

Our broker will provide us with the date of transfer and the high, low and close prices for that date. We will issue a tax receipt to you for the total value of the shares at the high price on the date of transfer.
