Adoption Follow-Up Survey

The PAWS staff would like to thank you for adopting a homeless animal. We hope the materials and information you received at the time of adoption have been helpful while your new companion settles into your home.

In an effort to improve our services, the following questionnaire is designed to discover how you felt about our adoption process so we can provide the best service to future adopters.

Please also feel free to share your story with us on our Facebook, Twitter and Instagram accounts, or by email. We love hearing updates and seeing photos of our adopted animals in their new lives, and we appreciate the feedback.

Please note: all fields marked with an asterisk (*) are required, and must be completed before the form will submit successfully. If you encounter any problems using this form, please email for technical assistance.

Thank you!

Did you remember to check that the microchip registration was correct? If not, please refer to page 4 of the adoption packet for simple instructions. Remember, a microchip is only good if the registration information is correct and up-to-date!

Did you remember to license your new pet? If not, please visit the city or county website where you reside to purchase a license. Citizens of Mountlake Terrace, Seattle and unincorporated King County can also purchase a license at PAWS. A properly fitted collar with identification and a registered microchip are the best ways for a lost pet to be reunited with their family.
